Care management that fits a real Alabama clinic - not a national call center.
Partners in Care is run by Kristina Loftis, ASHM, LPN, a licensed clinician who builds CCM, RPM, and RTM programs around how your independent practice, RHC, FQHC, cardiology, or PT/OT clinic actually works.

Sound familiar?
Your staff already wears five hats
Front desk, referrals, prior auth, patient calls - there's no open hour left to also run chronic care outreach and document every minute of it.
National vendors don't know your patients
A call center in another state reading from a script isn't who your patients want managing their care between visits - and it isn't who you want representing your clinic.
Getting a program off the ground is the hard part
Consent workflows, device logistics, and monthly time documentation stall a lot of CCM/RPM efforts before they ever help a patient.

Common questions
Can a patient be enrolled in CCM and RPM at the same time?+
Often yes - CCM and RPM are separate services and can be furnished to the same patient in the same month when each program's own requirements (time, consent, and documentation) are met independently. Whether it makes sense for a specific patient depends on their conditions and your clinical judgment, and your billing staff or biller should confirm code-level requirements before submitting claims.
Do RPM devices need home Wi-Fi?+
No. We use cellular-connected devices (blood pressure cuffs, glucometers, pulse oximeters, and similar) that transmit readings over the cellular network, so patients without home internet or Wi-Fi can still participate.
Does Alabama Medicaid cover RPM?+
Alabama Medicaid does have coverage pathways for remote patient monitoring, though covered codes, eligibility, and prior authorization requirements can differ from Medicare and commercial payers. We review your specific payer mix during the consult rather than assuming one set of rules fits every patient.
Who actually bills for CCM, RPM, or RTM once a patient is enrolled?+
Your practice bills under its own NPI - we're a care management partner, not a billing company standing between you and the payer. We track time and prepare documentation your billing staff needs; your practice (or your existing billing service) submits the claims.
Reimbursement depends on payer, patient eligibility, and documentation. We don't promise specific revenue or reimbursement amounts. See program details.
